Applying to Vauxhall

CONGRATULATIONS! Once your child/ward has earned a space at our noble institution, you will be given a package at the beginning of the school year to be filled out and returned to the Main Office. These documents are integral in maintaining records for the student’s tenure.

Below are the documents which are disbursed to parents/guardians:

1. Personal Data Sheet : Download
2. Medical Form : Download
3. Two Passport size photograph
4. Birth Certificate
5. Immunization Card

Please make every effort to make these available to us as soon possible, so we can further update your child’s record.

Please Note: If you are seeking admission to attend Vauxhall, the following forms must be completed and returned along with the following:

1. Transfer In Form : Download
2. Last Report Card
3. Two passport size photographs
